Care Journeys lets a practice define the phases that repeat across a program or care approach, then creates a client-specific copy when the journey starts. Providers can move the client manually, use measurable automatic rules for selected phases, and change direction without losing the history.

Choose a client, select a saved journey, and add an optional target date. The first phase starts immediately, and the client journey keeps its own copy so later template edits do not rewrite active care.
Review the active improvement plan, total and completed assignments, shared reflections, and completed check-ins recorded since the phase began instead of judging the phase from elapsed time alone.
For any non-final phase, choose manual movement or require every saved rule to be met: days in phase, completed assignments, assignment completion rate, shared reflections, and completed check-ins.
A provider can move forward, return to an earlier phase, skip ahead, or update the current phase note even when automatic rules are configured. The final phase is always completed manually.
The record shows when the journey started, phase changes, provider notes, automatic changes and the rules that triggered them, completion, or an early end, along with the recorded actor and time.
