Build the plan in layers

    Open Improvement Plan and confirm the correct client before adding information.

    Add a focus area

    Describe the client priority in clear, collaborative language. Avoid unnecessary sensitive detail.

    Add a strategy

    Record the approach that connects the focus area to a practical next step.

    Add an activity

    Choose a suitable activity, adapt its instructions, and set a due date or recurrence.

Review before sending

  • Check that every assignment is appropriate, understandable, and accessible to the client.
  • Remove duplicate or outdated activities and confirm timing with the client.
  • Use Approve & send to make pending assignments available in the client app.
Keep in mind

Approving a plan sends its pending assignments. It does not independently establish consent, medical necessity, or completion.

Keep the plan current

Review the plan after new client context, completed work, or a meaningful change. Continue, simplify, replace, or stop an activity based on the client discussion and professional judgment.
