Start with the question

Choose the decision or conversation the information should support. Select the client, signal, and time range that match that purpose.

Compare appropriate evidence

  • Use repeated mood or discrete tracking to view self-reported patterns over time.
  • Review assessment scores with measure guidance and administration timing.
  • Use shared journal or conversation insights as summaries that require verification with the client.
  • Review completion and messaging activity as engagement signals alongside the clinical outcomes selected for care.

Document the interpretation

Record the relevant trend, uncertainty, client perspective, and resulting care decision through the organization’s required workflow.

Keep in mind

Interpret chart changes alongside timing, life events, co-interventions, and client context.
