Guided journaling

Assign a focused prompt and respect the client’s journal-sharing choice.

4 min readUpdated July 2026

Write a useful prompt

  • Connect the prompt to an agreed care-plan purpose.
  • Use clear, open language and avoid asking for unnecessary sensitive detail.
  • State whether and how the entry will be discussed.

Assign the practice

  • Choose Journaling in the activity library.
  • Add or adapt the prompt, select timing, and approve the assignment.
  • Use recurring timing only when repeated reflection is appropriate.

Review with permission

Keep in mind

Clients control journal sharing. A missing journal view can mean sharing is off; it does not mean the client failed to journal.
