Discrete progress tracking
Track a selected wellbeing signal such as sleep, anxiety, or emotional stability over time.
Define the signal
- Choose one construct that is meaningful to the care plan.
- Explain the scale in client-friendly language.
- Avoid combining different concepts under one label.
Assign consistently
Use recurrence to create comparable observations and review the trend at an agreed interval. Change the cadence if repeated tracking becomes burdensome.
Interpret with context
Keep in mind
Review each discrete signal alongside timing, life events, care changes, and the client’s account of what changed.
