Treat every output as a review aid

  • Check the source date range and which information was available.
  • Look for missing sharing, sparse data, contradictory entries, or recent events not represented.
  • Confirm material statements with the client and appropriate records.

Respond to a concerning signal

  1. 1
    Review promptly

Do not wait for another automated run when the signal suggests timely attention.

  2. 2
    Assess directly

Use the provider’s established assessment process and current client contact information.

  3. 3
    Take appropriate action

Follow the organization’s safety, escalation, emergency, and documentation procedures.

Keep direct assessment central

Important

Assess current safety directly according to the practice’s process, whether or not an automated flag appears.
