Review Care Journeys
Use the client universe, journey stages, review flags, and check-in timing to organize follow-through.
Find the client who needs attention
- Use search to locate a client.
- Filter by All, Needs review, Check-in due, or No journey.
- Open a client to see the current stage, latest check-in, readiness, distress, and available journey history.
Read the journey
- Journey stages summarize recorded workflow events and automated runs.
- Thought Process shows the available rationale behind a run for provider review.
- Risk and human-review flags should prompt timely provider review according to the client context and established procedures.
Follow through
Use the next-check-in information and current context to decide on a message, assignment, plan update, direct contact, or other appropriate action.
Important
Use Care Journeys as an organizational view and follow the practice’s direct assessment and emergency procedures for safety decisions.
